Lists of personal military records (Appl 5)
Personal military registration in state bodies, local governments, enterprises, institutions and organizations is carried out according to the lists of personal military registration (Appendix 5) in accordance with clause 33 of Resolution No. 1487 of the Cabinet of Ministers of Ukraine dated December 30, 2022. Lists are formed separately in four categories:
- Officers,
- Private, sergeant, senior officers,
- Women,
- Conscripts.
Running Report Generation
The system provides the ability to generate the report in Word and Excel formats.
To generate the report in Word format, run the following report: Military Service Reporting / Lists os personal military records (Application 5)

When launching the report, it is needed to specify the date for which the report should be generated. Based on this date, the system includes only those employees who are active as of the selected date. Also, it is needed to select the checkbox for the category for which the lists should be generated.
If needed, additional filters can be added using the Add Filter button.
Include terminated employees from beginning of year - if this option is enabled, the report will include:
- active employees
- employees dismissed since the beginning of the calendar year
- employees removed from military registration since the beginning of the year
Military Dismissed Employees - the parameter allows generating a list of person whose military registration documents are stored separately. If the checkbox Military Dismissed Employees is enabled, Application 5 will include only such employees.
In the Person filter section, you can apply a filter by the field “Military Office” to generate a report for persons registered in a specific Territorial Recruitment and Social Support Center.
